Care Coordination For Children (910-671-3200)
Care Coordination for Children is a population management model in partnership with Community Care Networks and Local Care Management Entities, in addition to the community base interventions for children to maximize health outcomes, the program will target the highest risk and highest cost children for care management. Care management services are provided for all Medicaid children birth to 5 years of age who are determined to be high-risk and qualify for services.
In partnership with the Lumberton Children’s Clinic, the Child Health Plus Clinic provides comprehensive child health and wellness care for patients ages birth to 21 years of age. A physician is available each day to address the child’s health care needs. Services include well child care, checkups, preventative services, immunizations, sick visits and referrals to specialists, when necessary. Appointments are encouraged. Medicaid, private insurance and self-payment accepted. Sliding fee scale available based upon income eligibility.
The Child Health Plus Bus is a mobile clinic offering comprehensive health care services for children birth to 21 years of age. Services offered on the mobile health clinic include well childcare, sick visits, immunizations and check-ups. Also available are kindergarten health assessments and sports and cheerleading physicals. Self-pay, private insurances and Medicaid are accepted.
